Since you plan to do a cost-effectiveness analysis, you need a
measure that can be used as a utility. This rules out the SIP
and the MOS-HIV (and, of course, the MOS-HIV is specific to a
certain disease while you want a broader measure). The SG would
meet the need for a utility measure, but it is difficult to administer
to large numbers of people and is not well-suited to a mailed
survey. The EQ-5D has a simple format for respondents, and has
been used successfully in a mailed survey.
