Because you plan to do cost-effectiveness analysis, you need a preference-weighted measure. Of the ones shown, only the TTO is preference-weighted. Although another utility measure, such as the standard gamble, could be appropriate, considerations in your choice between TTO and SG may include the issue that the health state may be considered a minor disease by some respondents, and so you would like to have a measure that will detect small changes in utility reliably. The SG may be problematic since an accurate assessment of the utility would require the respondent to consider very small probabilities, which is quite difficult for many individuals (Woloshin, Schwartz et al., 2001). Time can be traded off in small units that are still familiar to most people, such as days or even hours.
